2026-2027 Early Enrollment Form
For use between December 5, 2025-January 16, 2026

INSTRUCTIONS
This form is exclusively for current Trinity Lutheran Church & School families.  The early enrollment fee is $250 per student.  The Early Enrollment Form and payment are due by Friday, January 16, 2026, 315pm HST.

Enrollment for new students begins January 17, 2026 at $600 per student.  

All current 2025-26 TLS students must complete this form.  If student/s are NOT returning for the 2026-27 school year, please complete sections A & E.  

Enrollment fees are non-refundable.

Enrollment fees hold your child/ren's spot in the class.  In the event you withdraw your child/ren before July 28, 2026; the balance of the standard enrollment fee will be collected.

Spots are filled on a first-paid, first-served basis.

Trinity Lutheran School reserves the right to deny enrollment to students who's academic and/or behavior standards do not meet our school's standards.  All current year financial obligations must be met before any student will be considered for enrollment.

B.  ADMINISTRATIVE PLANNING

For administrative planning purposes, please answer the following questions as best as you can.

We plan to enroll in Before &/or After School Care for the 2026-27 school year.  See the Fee & Tuition Schedule for more information.  Do not check this box if you will be using these services less than three times per week.

The undersigned accepts the objective and policies of Trinity Lutheran School as stated in this enrollment form and the TLS Parent/Student Handbook and will be financially responsible for the prompt payment of all fees and tuition.
