We are thrilled to welcome your child to Trinity Lutheran School for the 2026–2027 school year!
Now that enrollment is complete, we are moving forward with the registration process.
To finalize your child’s registration, please complete the following required forms:
You may submit your completed forms in one of the following ways:

By email: Send to admissions@tls-hawaii.org

In person: Print and return forms to the school office

On-campus assistance: Schedule an appointment with Mrs. Flores, cflores@tls-hawaii.org, to complete the forms on campus
All forms are due by May 29, 2026. Please note that a $100 late fee will apply to any registrations submitted after this date.
Medical Requirements
For new students entering Trinity and all incoming Kindergarten and 7th grade students. Medical requirements must be completed by May 29, 2026.
A Student Health Record is required, including TB clearance, and immunization record. The student's physical must be completed no earlier than July 1, 2025. Download the form here:
Additionally, new Preschool and PreKindergarten students are required to complete the DHS 908 Supplement form. Download the form here:
